Wings for Autism® Repeat Participant / Waiting List Registration

Please complete this registration form if you have previously attended a Wings for Autism event at Phoenix Sky Harbor International Airport (PHX) and are interested in attending again OR if you have been redirected here to register for the Waiting List.

Repeat Participants: Priority will be given to first-time families for this event. However, if there is additional space we will invite repeat families to attend again. We appreciate your understanding of our effort to give all families the opportunity to practice visiting the airport.

Waiting List Registrants: Families on the Waiting List will be contacted, in order of date of registration, as openings for this event become available. While we cannot guarantee space will open for all Waiting List families, we will do everything possible to accommodate everyone who wishes to take part.

Wings for Autism at PHX will take place on October 7, 2017. Participants will be boarding the aircraft at 6:00 PM, but we ask that you arrive at the airport no later than 5:00 PM to allow ample time to check-in and pass through security. All participants 18 years old and over are required to bring a valid form of government-issued ID to be allowed through security.

Due to limited space on the airplane, a maximum of four (4) participants are allowed per family group, including the individual(s) with disabilities and family members/support staff accompanying them to the event.

T-shirts will be provided for participants with disabilities and family members/support staff.
