This form allows members to:
Promote OCA accounts to DPP
Request updates to contact information
Request updates to ticket delivery subscriptions
Submit AOI files for upload
Request information on membership
Contact the Member Services Department for questions and troubleshooting
To create a new user account, please complete the following steps:
 
Create an account at OneCallAccess. This must be done first.
 
Once this has been completed, complete this Member Service Support form for "Promote My OCA Account to the DamagePreventionPortal". Applicants will need to include which type of user role, or "access", they require. A chart is below for reference.
 
Upon receipt of the promotion request, the Member Services Department will set up the account and follow-up via email. 
Days of the Week Applied to the Subscription
If new subscription is left blank, it will be applied for all days. If subscription change is left blank, we will assume there is no change to the days applied.
Affected Ticket Types for the Subscription
If the new subscription is left blank, all will be assumed. If the subscription change is left blank, we will assume there is no change in ticket types. If this is an IVR subscription it will apply to emergency ticketes.

Requirements for File Upload
 
- Zipped file format
- Zipped file must include the file extensions of DBF, PRJ, SHP, and SHX
- Datum used must be NAD83 or WGS84
 
One file upload per submission.

Important
Once the SHP file upload is complete, an email from the Member Services Department will be sent to the address provided. Please be ready to log into the DamagePreventionPortal to view, approve, and publish your new AOI. It is critical to publish the placement as soon as possible to ensure that your utilities are covered within the MISS DIG 811 System.
