COVID Vaccine Scheduling

Please fill out the form below to schedule an appointment for your first, second or booster of the COVID Vaccine. 
 
First and second doses (Pfizer) are available to anyone age 5 and older.  There must be at least 21 days in between your first and second dose.
 
Pfizer 1st booster (third dose) vaccinations are available to individuals 12 and older, who have completed their primary COVID-19 vaccination series at least five months ago.  Click here to see a full list of those that are eligible.
 
Moderna 1st booster (third dose) vaccinations are available to individuals 18 and older, who have completed their primary COVID-19 vaccination series at least five months ago.   Click here to see a full list of those that are eligible.
 
Pfizer and Moderna 2nd booster (fourth dose) vaccinations are available to individuals 50 and older or individuals 12 and older who are moderately or severely immunocompromised. Click here to see a full list of those that are eligible.  At this time a 2nd booster (fourth dose) is not recommended for anyone receiving the Pfizer vaccine under the age of 12 or anyone receiving the Moderna vaccine under the age of 18.
 
If you have received a COVID infusion you must wait 90 days before getting a booster shot.

Moderna Booster Vaccines:
1st booster vaccines are approved for anyone 18 and older and 2nd booster doses are approved for anyone age 50 or older or if you have a qualifying health condition.  At this time, if you are under 18 you are not eligible for a booster vaccine.
