The purpose of this Quality Assurance Form is to enable the centre to reflect on their delivery of FHT Accredited Short Courses and Qualifications and to improve the quality of service. This form must be completed annually prior to the accreditation renewal and submitted to the FHT within 10 working days of receipt.
This Quality Assurance Form is to be used in conjunction with the FHT Accreditation Terms and Conditions. We ask that you please read through all information carefully before completing this form.
Data protection
FHT adheres to General Data Protection Regulation (GDPR), which applies from 25 May 2018. Your information will only be used for accreditation purposes.
The information you supply on this Formsite form will be securely transferred outside of the European Economic Area (EEA) in order to process this form.
We will share your information with our FHT Accreditation Inspector(s) who will review your course information against any applicable education standards and, if you are applying for accreditation of a qualification, conduct any inspections of your training premises.
