I hereby apply for a Sewer Main Cut-in to be undertaken by Council on the above property and acknowledge and agree that:

• A mains tapping cut-in fee is payable as per Council's Standard Fees & Charges;

• A sewerage charge is payable per annum upon connection to mains; and

• The plumbing facilities connected to the Council Sewerage Main are maintained and used in accordance with the requirements of the Water Act 2000, as amended.
