Capital Music Center Lesson Exit Form

Thank you for participating in our lesson program. We are sorry to hear that the time has come for your lessons to stop. Capital Music requires a reasonable notice* that provides the fairest amount of time to both student and teacher, as this gives each the time to give closure, adjust schedules, and end lessons on the most positive note possible.

*Minor Students: Parents & Student agreed to try lessons for at least 4 months. Withdrawal from lessons after that simply requires a 30-day notice.

*For Adult Students: You agreed to try lessons for at least 1 month. Withdrawal simply requires a 30-day notice.

*For Group Students: You agreed to commit your child to lessons for the remainder of the school year (May 31). {Exception: You may give notice by Dec 1 to drop the upcoming Spring Semester (Jan-May)}.
