ENROLLMENT REQUIREMENTS:
1. Payment Options:
- Option 1: Pay on-line with Paypal or a credit card. If you pay on-line, you must pay in full.
- Option 2: Pay by check. If you pay by check, you may pay the full amount or make a minimum
payment of $250. You must mail the check along with a copy of this application to the address shown at the
top of this form. No student will be assigned to a class until an application and at least the minimum payment
have been received.
2. Students must be 16 years or older by the end date of the class. A copy of a birth certificate is required.
3. Students seeking enrollment should be motivated and serious about completing the requirements of this course. This includes both the
classroom and
driving components.
4. Students must be free of any disability (physical or mental) that would interfere with the safe operation of a motor vehicle.
5. Students must not be under any suspension or revocation which would prevent him or her from obtaining a license.
